Job Title: Independent Living Assessor / Advisor 

Location: Cambridgeshire – Home based with possibility of travel (planning own diary) – Mileage paid from home.

Salary: up to £23,400 

Type: Permanent – Full Time 

Essential Criteria:

• Experience of working within a Finance or Accounting setting would be ideal, as calculating care / support payments, costings and creating support plans is an important part of this role (any sector).
• Experience of working within the social care sector, supporting Children and Adults with disabilities to live independently in their own homes / communities.
• Experience of operating online systems, processes and databases – this role involves utilising multiple online systems and portals so IT skills are essential.
• Ability to overcome barriers and work with challenging behaviour.
• Driver with own vehicle.

Duties:

• The successful candidate will be responsible for providing high quality information, advice and support to eligible customers who are referred, or are considering self-referral for Support Schemes, Independent Living Schemes, Personal Health Budgets and self-funded/private clients.
• Deliver a person-centred service that enables individuals to focus on the outcomes they wish to achieve; supporting with establishing a care and/or support plan that helps the individual meet these outcomes in a cost-effective and safe manner.
• Managing your caseload and arranging home visits with customers in order to make sure they have all the support they need to retain their independence.
• Obtaining and collating information, maintaining accurate records and utilising databases and online systems
